Article 01

Understand the patient and client app

The PrecisionMind app and web portal connect a patient's or client's day-to-day activity with their clinical care plan.

Progress and assessments

Automated scoring: Assigned clinical measures are completed in the app or portal and plotted on the clinician dashboard.
Mood and coping logs: Patients and clients can record daily reflections on mood and the effectiveness of strategies discussed in session.
Intake and history: They can complete digital forms and history-gathering tools before or between appointments.

External medical updates

Health events: View notifications for emergency department visits or updates from other medical providers through HIE networks.
Medication changes: Review new prescriptions or changes made by the broader care team.
Care alignment: Use shared data to coordinate the treatment plan with the patient's or client's overall medical care.

Activity and sleep patterns

Wearable syncing: Patients and clients can connect Apple Health, Google Fit, or Fitbit data in the app.
Objective trends: Review trends in sleep, daily movement, and activity levels.
Privacy controls: Patients and clients choose which data categories to share and can disconnect at any time.

Communication and appointments

Secure messaging: Share text, photos, and files in a HIPAA-compliant space.
Telehealth: Patients and clients can join video sessions from mobile, web, or desktop. Email reminders include the session link.
Scheduling: They can view upcoming appointments, review past visits, or self-schedule when enabled.

Article 02

Help patients and clients get started

Share these steps with patients and clients so they can set up PrecisionMind and connect their account to your clinician dashboard.

Set up an account

They must use the email address you have on file for them.

To set up an account:

Download PrecisionMind from the App Store or Google Play. , or open the client web portal.
Tap Start my journey .
Enter the email address on file with your practice.
Enter the 6-digit verification code sent to that email address.
Create a password.

Complete the profile

PrecisionMind asks for details used to personalize care and keep clinical and insurance records accurate:

Identity: Full legal name and date of birth.
Contact: Phone number and home address.
Personal details: Gender identity, preferred pronouns, and the sex or gender information required for insurance.

Choose app settings

During setup, patients and clients can choose:

Wellness sync: Connect an Apple Watch, Fitbit, or Google Fit to share sleep and activity trends.
Notification preferences: Receive appointment reminders and messages by push notification, email, or SMS.
Easy access: Enable Face ID or Touch ID for secure access.

Note: Wellness sync is available in the app only.

Use the Home screen

After setup, patients and clients arrive at the Home Screen .

From the Home screen, they can:

Join telehealth: Enter virtual sessions from the app or portal.
Message securely: Send updates or files in a HIPAA-compliant space.
Complete tasks: View and complete assigned assessments or forms.

Support and privacy

Data control: Patients and clients can stop sharing specific data categories or disconnect devices at any time.

Article 03

Understand the patient app Home screen

The Home screen gives patients and clients one place to find check-ins, appointments, and pending tasks for the day.

Daily check-ins

Check-in prompts appear at the top of the screen and encourage reflection throughout the day.

Frequency: Check-ins are available in the morning, afternoon, and evening.
Flourishing Scale: These reflections help patients and clients track how they are feeling.
Clinical insight: Responses are shared with the clinician dashboard to provide an ongoing view of progress.

Appointments and telehealth

Current and upcoming appointments are easy to access.

Join: Opens a virtual session.
Get Directions: Opens directions to the office for an in-person visit.

Tasks and assessments

The "Before Your Next Appointment" and "Pending Assessments" sections show required actions.

Practice forms: Open fillable forms or intake paperwork.
Assigned measures: Review the assessment title, estimated time, and due date.
Results: Completed assessment results are sent to the clinician dashboard immediately.

Article 04

Check-ins and the Flourishing Scale

Check-ins help you reflect on your emotional and mental well-being throughout the week.

How check-ins work

You receive three check-ins per day .
Check-ins appear every day.
You can complete morning, afternoon, or evening check-ins, depending on the time of day.
Each check-in uses the Flourishing Scale questionnaire .

Complete a check-in

To complete a check-in:

Tap Check-in with yourself .
Answer the questions.

After you finish:

You receive a personalized message based on your score.
Your responses are saved securely.
Your clinician can view your trends and progress over time.

Missed check-ins

If you miss a time slot, the next check-in replaces it automatically. For example, if the morning window has passed, you will see the afternoon check-in.

Article 05

Help patients view, join, and manage appointments

Patients and clients can use the Appointments tab to review upcoming, cancelled, and completed visits.

Find appointments

Appointments appear in two places:

Home Screen: Shows confirmed upcoming sessions.
Appointments Tab: Organizes sessions into Upcoming , Cancelled , and Completed .

Attend a session

Available actions depend on the appointment type:

Telehealth Sessions: Use Join Button to enter the virtual room, or Copy Link to open the session on another device.
In-Person Visits: Review the clinic address and use Get Directions to open turn-by-turn directions.

Manage requests and updates

Pending Appointments: A proposed time appears as a request. The patient or client can select Accept or Decline , which updates the clinician dashboard.
Red Dot: Appears on an appointment card when details change.
Notification Counter: Appears on the Appointments icon when there is new activity.

Note: For telehealth issues, suggest checking the connection or using Copy Link to try another device.

Article 06

Message your care team

You can message your care team from the PrecisionMind app or portal to ask questions, share updates, or send documents.

Open chat

To open a conversation:

Tap the Chat tab in the bottom navigation.
Select a care team member.

All care team members appear in the list, even if you have not started a conversation with them.

Send a message

Type and send a text message in the conversation.

Send an attachment

To send an attachment:

Tap the "+" button .
Choose a document, a photo from your gallery, or an image from your camera.
Send the attachment.

Attachments are visible to your clinician and remain in the conversation history.

Message notifications

A new message can appear as:

An indicator dot on the Chat tab.
An unread indicator on the conversation.
A push, email, or SMS notification, depending on your permission settings.

Chat availability

Chat is for non-urgent communication. Response times depend on your care team's availability.

Important: For urgent medical concerns, follow the emergency instructions provided by your clinician.

Article 07

Connect a wearable and view activity data

The Activity page shows health trends from a connected wearable. This feature is available in the app only.

Connect a wearable

To connect a wearable:

Open the Track tab.
Tap Activity .
Tap Connect a wearable .
Browse or search for a supported device.
Tap Connect for your device.
Authenticate in the wearable's app.

Data begins syncing automatically.

View your data

Graphs show weekly trends for:

Sleep duration.
Sleep onset and wake-up times.
REM sleep duration.
Total daily steps.
Resting heart rate.

Each graph also includes a tip.

Manage devices

To manage connected devices:

Tap the settings icon on the Activity page.
Review the list of connected devices.
Tap Disconnect for a device to stop syncing its data.

Article 08

View and complete forms and documents

PrecisionMind supports forms you sign, read-only documents, and forms you fill out.

Access your forms

To access your forms:

Tap My Forms from Home or Profile.
Choose Pending or Completed .

Form types

Sign form: Read and sign the form as required.
View document: Read the document without completing any fields.
Fillable form: Answer multiple-choice, short-answer, or long-answer questions.

Completed forms generate a PDF for you and your clinician.

Note: You can download forms at any time from the Profile tab.

Article 09

Manage your profile: Payment, emergency contact, and settings

Your Profile page lets you update your personal details, medical information, and account settings.

My Care Team

This section shows everyone involved in your care, including:

Each team member's name.
Their role.
Their contact information.

Manage section

From this section, you can:

Emergency contact: View or update the person PrecisionMind should contact in an emergency.
Payment details: Add insurance information or set up private payment.
My forms: View pending or completed forms your clinician shared with you.
Manage permissions: Adjust data sharing, notification, and Face ID settings.
Change password: Update your password.

Others section

Email: Open your email app to contact PrecisionMind support.
Rate & Review: Open the app store page to share feedback.
Terms & Conditions and Privacy Policy: Open the policy documents for review.

Article 11

Delete your account

You can request permanent deletion of your PrecisionMind account from the patient app.

To request account deletion:

Open the Profile tab.
In Edit Profile, scroll to Delete My Account .
Tap Delete My Account to view the instructions.
Email support@PrecisionMind.health to request account deletion.

Important: Account deletion is permanent and cannot be undone.
