Assessments in PrecisionMind help clinicians track client symptoms, monitor progress, and measure treatment responses over time - all within a single, structured space.

Article 01

Send an assessment to a client

Use Standardized Assessments to send a structured measure to a client.

Choose an assessment

To choose an assessment:

Open the Medical tab on the patient's profile.
Open the Assessments tab.
Select Send Assessment .
Use the Search bar to find an assessment, if needed.
Select the assessment.

Each assessment card shows its name and short form, number of questions, description, and average completion time.

Preview the assessment

To review the questions before sending:

Select Preview .
Review the assessment and its questions.
Select Select Assessment .

You can also select Select Assessment directly from the assessment list. Both paths open the scheduling flow.

Set the schedule

To finish scheduling:

Set the completion deadline.
Choose non-recurring , weekly , bi-weekly , monthly , or quarterly .
Add an optional note for the patient.
For a recurring assessment, choose when the recurrence ends.
Select Send Assessment .

The patient receives the assessment under the Home tab in the PrecisionMind app or portal, along with an email notification.

Article 02

Schedule recurring or future assessments

Schedule and repeat assessments to track patient progress consistently.

Non-recurring assessments

For a non-recurring assessment, choose the deadline by which the patient should complete it. PrecisionMind sends one request and does not repeat it.

Recurring assessments

For an assessment the patient should complete regularly, choose a recurrence frequency:

Weekly.
Bi-weekly.
Monthly.
Quarterly.

Then choose when the schedule ends:

On a specific date: Choose the final date.
After a specific number of occurrences: Choose between 2 and 12 occurrences.
Never: Continue the selected frequency without an end date.

After configuring the schedule, send the assessment. PrecisionMind sends each occurrence according to that schedule.

Reschedule one recurring assessment

To move one occurrence without changing the rest of the schedule:

Open the Pending or Overdue assessment.
Select Reschedule .
Choose the new date.

Only that occurrence moves. The remaining occurrences keep the original schedule.

Article 03

How clients receive and complete assessments

Clients receive a notification in the PrecisionMind app or portal when you send an assessment.

Complete an assessment as a client

To complete an assessment:

Open it from the Home screen .
Answer the questions.
Submit the assessment.

After submission, the assessment syncs to the clinician portal and its status changes to Completed .

Complete the assessment in one attempt

Assessments cannot be partially saved. After starting an assessment, the client must either:

Complete and submit it.
Select Discard to remove unsaved responses.

Complete an assessment for a patient

For a Pending or Overdue assessment, a clinician can:

Select Fill for Patient .
Answer the questions with or for the patient.
Select Complete Assessment .

This can be useful when you complete an assessment together during a session or call.

Important: A patient and clinician cannot complete the same assessment at the same time.

Article 04

Track assessment progress and results

Use the Assessments tab in a patient's profile to track assessments and manage requests you already sent.

For each assessment, you can view:

Assessment name and short form.
Status.
Scheduled On date.
Due On date.

Assessment statuses

Pending: The assessment was sent but has not been completed.
Completed: The assessment was submitted.
Overdue: The assessment was not completed by its deadline.

Select a Pending or Overdue assessment to see management options.

Reschedule an assessment

To reschedule an assessment:

Select a Pending or Overdue assessment.
Select Reschedule .
Choose a new date.

A non-recurring assessment moves to the new date. For a recurring assessment, only the selected occurrence moves and the remaining schedule stays unchanged.

Cancel an assessment

You can cancel Pending or Overdue assessments.

Non-recurring assessment: Select Cancel Assessment . PrecisionMind removes the assessment from both the patient's app and the clinician's assessment list.
Recurring assessment: Select Cancel this instance to cancel only the selected occurrence, or Cancel all incomplete instances to cancel every incomplete occurrence.

Complete an assessment for a patient

To complete a Pending or Overdue assessment for a patient:

Select Fill for Patient .
Enter the patient's responses.
Select Complete Assessment after answering every question.

Important: Assessments cannot be partially saved. If you leave before completing the assessment, select Discard to remove your changes.

Only one person can complete an assessment at a time. The clinician and patient cannot fill out the same assessment simultaneously.

View completed assessment results

To view results, select a Completed assessment record.

Depending on the assessment, you can review:

Submission date.
Responses and results.
A graph of scores over time, when available.
Automated interpretation powered by PrecisionMind Insight, including trends and context from previous results.

Article 05

Review self-reports

Use the Self-Report tab to review a client's Flourishing Scale Report submissions.

Review self-reports

In this tab, you can:

View all self-reports and their submission dates.
Open a report to review the client's responses.
Review the total flourishing score and submission time.

Clients can submit self-reports at any time, giving you ongoing insight between structured assessments.
