Article 01

Manage form templates

Use form templates to create, upload, and organize the forms your practice sends to patients.

Open form templates

To open your form library:

Select Management from the left sidebar.
Select Manage Form Templates .

What you can access depends on your permissions:

My Forms: Create forms, rename or delete your own forms, and send them to patients.
Other Forms: View and send templates created by other practice users when your permissions allow. You cannot edit or delete another user's templates.

Template details

Each template shows:

Form Name: The internal name used when selecting forms.
Date Uploaded: The upload date, which can help you track versions.
Type: PDF - To View, PDF - To Sign, PDF - To Fill, or Dynamic Form.
Actions: Options to rename or delete your own templates.

Create a form

Select Create New Form , then choose Upload Viewable PDF , Upload Fillable PDF , or Create Dynamic Form .

Upload a viewable PDF

Use a viewable PDF for practice policies, HIPAA notices, educational material, or consent documents that only require a signature.

To upload one:

Select Upload Viewable PDF .
Upload a PDF up to 50 MB.
Choose whether the form requires a patient signature.
Save the template.

If you require a signature, the patient must sign digitally before submitting.

Upload a fillable PDF

Use a fillable PDF for government forms, insurance paperwork, or other existing documents with interactive fields.

To upload one:

Select Upload Fillable PDF .
Upload the PDF.

PrecisionMind checks for interactive fillable fields.

No fields detected: The upload stops and PrecisionMind asks you to upload a valid fillable PDF.
Fields detected: Choose whether a signature is required, then save the template.

Patients complete the fields in the PDF while its original layout is preserved.

Create a Dynamic Form

Use a Dynamic Form for intake questionnaires, medical history, screening tools, custom assessments, or structured data collection.

Add questions using Short Answer, Long Answer, Single Choice, Multiple Choice, or Rating Scale fields.

Turn on Required for questions that must be completed before submission. Save the template when finished.

Best practices

Use Dynamic Forms for patient intake. Structured responses can be referenced throughout PrecisionMind and support AI-powered workflows.

Use fillable PDFs for existing paperwork. Upload standardized PDFs with interactive fields instead of rebuilding them.

Use clear names. For example: Practice Name - Informed Consent - 2026 .

Test before using. Create a test patient with your own email address to experience the workflow as a patient.

Article 02

Send a form to a patient

You can send forms during patient onboarding or later from the patient dashboard.

Send forms during onboarding

When you invite a new patient, forms configured as Default Forms are selected automatically. Before sending the invitation, you can remove a default form or add another form from your library.

After the patient creates an account, the forms appear as tasks in the PrecisionMind Patient App or Portal and remain visible until completed.

Send a form from the patient dashboard

To send a form:

Open the patient's record.
Open Medical Data .
Open the Forms tab.
Select Send Form .

Select a form

You can choose from:

My Forms: Forms you created.
Other Forms: Forms shared by other users in your practice. You can send these when your permissions allow.

Use the search bar to find a template.

Note: If the form is not in your library, select Upload . Upload a PDF up to 50 MB, rename it for the patient's view, and select ✅ Check when a signature is required.

Preview the form

Select the form, then select Preview to review it before sending.

Set an optional deadline

Select Next .
Choose whether the form should have a deadline.
No: Send the form without a due date.
Yes: Choose a future date. The patient sees this date when completing the form.

Send the form

Review your selections, then select Send . The patient receives the task immediately in the PrecisionMind Patient App or Portal.

Track completion

After sending a form, its status appears in the patient's Forms section. Completed forms are stored automatically in the patient's Medical Data.

Article 03

Sign and complete forms

How a patient completes a form depends on its type.

Viewable PDFs

The patient reviews the document and signs it before submitting when a signature is required.

Fillable PDFs

The patient completes the interactive PDF fields and signs before submitting when required.

Dynamic Forms

The patient answers questions built in PrecisionMind and submits the completed form.

Complete a Dynamic Form for a patient

You can complete a Dynamic Form for a patient during an appointment, enter responses collected verbally, or help someone who cannot complete the form independently.

To complete a Dynamic Form for a patient:

Open the patient's Dynamic Form.
Select Fill for Patient .
Enter the responses.
Save your progress to continue later, or submit the completed form.

Important: To prevent conflicting edits, the clinician and patient should not complete the same Dynamic Form at the same time.

Digital signatures

For a form that requires a signature, the patient:

Opens the form.
Reviews or completes the required information.
Signs electronically.
Submits the form.

The form status then changes to Completed .

Article 04

Track forms and manage deadlines

Every form sent to a patient appears in the Forms tab of the patient's Medical Data.

For each form, you can see its name, type, sent date, deadline when assigned, and current status.

Form statuses

Pending: The form was sent but has not been started.
In Progress: The patient started the form but has not submitted it. Dynamic Forms save progress automatically.
Completed: The patient completed the form.

Completion means:

Viewable PDF: The patient reviewed the document and signed it when required.
Fillable PDF: The patient completed the PDF fields and signed it when required.
Dynamic Form: The patient or clinician submitted all required responses.

Manage a sent form

For a Pending or In Progress form, select the ⋮ (More Actions) menu.

Reschedule Deadline

To change the deadline:

Select Reschedule Deadline .
Choose a new future due date.

The patient sees the updated deadline immediately.

Cancel Form

To withdraw an incomplete form, select Cancel Form .

The patient can no longer access it.
The patient cannot complete it unless you send it again.

Note: Completed forms cannot be cancelled.

View a completed form

Select a completed form to:

View the submitted form.
Review patient responses.
View digital signatures, when applicable.
See the signature date and time.
Download the document.

Notifications

PrecisionMind sends notifications when a form is sent and when a patient completes it.
