Article 01

Accessing treatment plans

Open the Treatment Plan tab to find draft and published plans for a patient.

Open treatment plans

To view a patient’s plans:

Open the Patient Dashboard .
Select Treatment Plan .
Select a plan from the list.

Understand plan status

Draft: The plan remains editable and shows its last updated date.
Published: The plan shows its publication date and is a locked clinical record.

Note: You can use the trash icon to delete a draft. Deleting a published plan may depend on your permissions.

Article 02

Creating and managing treatment plans

Create a treatment plan manually or start with an AI-generated draft from a transcribed appointment. In both cases, review and sign the plan before it becomes part of the clinical record.

In this guide, you’ll learn how to:

Start a treatment plan.
Review and edit its clinical sections.
Publish and share the plan.
Create updated versions over time.

Open treatment plans

To start a plan:

Select Patients .
Open the patient.
Select Medical Data .
Select Treatment Plans .
Select Create .

The list also shows each existing plan’s title, author, and publication date.

Choose how to create the plan

Start from scratch

Choose From Scratch to enter the plan manually. This option is useful for a new intake, when a transcript is unavailable, or when you prefer to write the plan yourself.

Start from notes

Choose From Notes to create a draft from a previous appointment. At least one session transcript must be available.

Select the appointment that is most relevant to the plan. PrecisionMind can use available information from:

Session transcripts and notes.
Clinical assessments.
Patient self-reports.
Electronic health record information.
Available sleep, activity, or wearable data.

Important: PrecisionMind does not diagnose the patient, choose treatment, or publish the plan. Review, edit, and sign all AI-generated content.

Review the clinical sections

The guided plan includes sections for:

The case summary and presenting symptoms.
Biopsychosocial history and patient strengths.
Diagnosis and differential diagnosis.
Assessments and measures of progress.
Clinical problems, goals, objectives, and interventions.
Recommendations, care coordination, risk assessment, and safety planning.

Review the proposed content, edit the language, add missing information, and select or deselect items. Then save and continue to the next section.

Review the complete plan

The Review screen shows the complete treatment plan. Confirm that the clinical information supports the diagnosis, goals, interventions, measures of progress, and current risk information.

Select Edit beside a section to return to it.

Add comments

Use the Comments panel to ask questions or coordinate with clinicians involved in the patient’s care. Comments do not change the plan’s clinical content.

Publish and sign the plan

To publish the plan:

Select Publish .
Type your name or draw your signature.
Complete the required confirmations.
Select Publish again.

Publishing signs and timestamps the plan.

Share the plan

Select Download PDF to save a copy.
Select Send to Patient to share a read-only copy through Messages in the PrecisionMind patient app.

Update the plan

When treatment changes, edit the published plan. PrecisionMind saves the update as a new version and keeps the original.

You can move between versions to review how symptoms, diagnoses, goals, interventions, or risk information changed over time.

Article 03

Creating a treatment plan with AI

PrecisionMind can create a draft treatment plan from information in the patient’s record. Review and edit every section before publishing it.

Requirement: The patient’s record must contain at least one session transcript.

Start an AI treatment plan

To start a plan:

Open the Patient Dashboard .
Select Treatment Plan .
Select + New Treatment Plan .
Select From Notes .
Choose the most relevant previous appointment.

Review the information PrecisionMind uses

PrecisionMind can prepare the draft from available clinical information, including:

Transcripts: Symptoms and themes discussed during sessions.
Clinical assessments: Scores from tools such as GAD-7 or PHQ-9.
Patient data: Available self-reports, sleep data, and activity data.

Review and edit the draft

To review the plan:

Check each section for accuracy.
Edit the text or add missing clinical information.
Select Save & Continue to move to the next section.

Sign and publish the plan

Review the complete plan in the consolidated view. Confirm that the interventions address the identified problems, then select Publish to sign and timestamp the record.

Article 04

Publishing and sending a treatment plan

Publish a completed treatment plan to sign and timestamp the clinical record. You can then download it or send a read-only copy to the patient.

Publish a treatment plan

To publish a plan:

Open the patient’s Treatment Plan tab.
Select the draft plan.
Select Publish .
Type your full name or draw your signature.
Select both confirmation boxes.
Select Publish again.

The plan becomes a signed and timestamped clinical record.

Send the plan to the patient

After publishing, choose one of these options:

Download PDF: Save a copy for offline review, an audit, or another record.
Send to Patient: Send the plan through Messages in the PrecisionMind app.

The patient receives a notification and can review the plan. They cannot edit the clinical record.

Update a published plan

When you edit a published plan, PrecisionMind asks you to save the changes as a new version. The original version remains available.

Use Version to move between versions and compare changes to goals, symptoms, or other sections.

Article 07

Reviewing and editing treatment plan versions

Open a treatment plan to update the current version or review earlier versions.

Open a treatment plan

To open a plan:

Open the patient’s profile.
Select Treatment Plans .
Select the treatment plan.

PrecisionMind opens the latest version by default.

Edit the current treatment plan

To update a plan:

Open the section you want to change.
Select Edit .
Update the goals, objectives, interventions, or other clinical information.

View an earlier version

To review version history:

Go to the top of the treatment plan.
Select Version .
Choose a version from the chronological list.

Each version remains available, so updates do not overwrite the earlier documentation.

Tip: Review previous versions before making a meaningful change to a diagnosis, goal, or intervention.
